Case Manager
Overview of the Position
CORE’s Short-Term Family Housing (STFH) seeks a Case Manager responsible for providing counseling services, conducting intakes with clients.
Essential Duties
• Provide concrete and counseling services to clients
• Conduct client intake
• Develop Family Housing Stability Plan as required by program
• Identify benefits or entitlements that may be available to the client
• Community outreach to businesses and other organizations that may provide resources to clients
• Meet with the clients as required, to evaluate progress and assist client in developing and meeting individual goals
• Provide necessary referrals for services, including mental health and/ or substance abuse counseling, as required
• Advocate on the client’s behalf when necessary
• Maintain data collection and case records as required by applicable rules and regulations
• Assist clients regain and maintain control over their lives and over their own transition to independent living
• Facilitate development of self-help skills, support systems, coping strategies, and increased hope and self-esteem that are central to the process
• Develop and present group/workshops for clients to facilitate
• Ensure clients adhere to the facility’s rules and regulations and other duties as assigned by the Program Director or designee
Necessary Skills
• Experience providing case management services including developing an individual service plan for at risk populations
• Familiarity with community resources
• Understanding of challenges facing homeless populations
• Consistently maintain required paperwork
• Ability to establish and maintain trusting relationships (having patience & perseverance)
• Ability to develop and maintain appropriate boundaries
• Must demonstrate sensitivity to needs of clients in crisis
• Must demonstrate responsiveness to cultural differences and a commitment to the value of cultural competency
Qualifications
• Bachelor’s Degree in social work, psychology, or related social service field required
• Minimum two (2) years experience working with homeless populations as a Case Manager/Case Worker
• Familiarity with HMIS and CATCH
